Home Medicines Reviews - What Happens and Who Qualifies?

A Home Medicines Review (HMR) is a GP-referred review of your medicines conducted in your home by an accredited pharmacist. The service is funded by the Australian Government for eligible patients and aims to identify medicine-related issues, answer questions and provide recommendations to your GP.

  • Take multiple regular medicines
  • Have recently been discharged from hospital
  • Have had changes made to their medicines
  • See multiple healthcare providers
  • Experience side effects or medicine-related concerns

Your GP will determine whether a referral is appropriate for your circumstances.

Who should ask about a Home Medicines Review?

The process usually begins with a referral from your GP.


After referral:

  • An accredited pharmacist arranges a visit to your home
  • Your medicines are reviewed in detail
  • Questions about your medicines can be discussed
  • A report is prepared for your GP


The review includes prescription medicines, pharmacy medicines, vitamins, supplements and other health products you may be taking.

How does a Home Medicines Review work?

No GP appointment needed for many conditions.

A home visit allows the pharmacist to see how medicines are stored and managed in everyday life.

No. A MedsCheck is conducted in the pharmacy, while a Home Medicines Review is a more comprehensive review conducted in your home following a GP referral.


How long does the visit take?

The length of the review varies depending on the number of medicines being discussed, but most appointments take around an hour.
